　　請假條格式：

　　標題：居中寫(xiě)《請假條》，用來(lái)表明此文是用來(lái)請假的。

　　稱(chēng)呼：平時(shí)怎么稱(chēng)呼，這里就怎么寫(xiě)，后面加冒號。這個(gè)地方往往是領(lǐng)導、相關(guān)負責人等。

　　正文：請假理由。直接寫(xiě)請假的原因，起止時(shí)間。如果有什么事要交代，也可以在寫(xiě)完請假原因之后接著(zhù)寫(xiě)要交代的事情。正文寫(xiě)完后要加祝頌語(yǔ)，祝福對方，這個(gè)是所有公文里表示對對方的友好。

　　署名：寫(xiě)在正文下右下方第三行處。如果是學(xué)生向老師請假，在名字前面加上“學(xué)生”二字。
